Self-employed doctor contributions simulator in France

Self-employed doctor in France: what will you keep once contributions and tax are paid? The official Urssaf simulator starts from your fees and expenses and calculates contributions, net income and income after tax.

A self-employed doctor in France, established or locum, pays contributions on non-commercial profit (BNC), including health insurance, family allowances, pension with the CARMF, CSG and CRDS.

These contributions weigh heavily on disposable income: it is worth estimating them before an investment, a new practice or a change of sector.

What the simulator calculates

  • Detailed social contributions
  • Net income, before tax
  • Income after French income tax
  • Monthly or annual figures
  • The effect of different fee and expense assumptions

The simulator covers practice in your own name (non-commercial profits). It does not cover professional practice companies (SEL). Medical Council fees are not calculated: add them to your expenses.

This official simulator is provided by Urssaf, the French social security agency (mon-entreprise.urssaf.fr). It loads from its website when you start it.

Indicative result, based on the figures entered and the rules known to date. It is no substitute for professional analysis: check your situation with a chartered accountant before deciding.

What next?

The simulator gives an order of magnitude. Bookkeeping, actual expenses, the 2035 return and a possible move to a practice company need year-round follow-up. See healthcare professionals

Frequently asked questions

Does the health insurance fund pay part of the contributions?

Yes, for doctors under the national agreement (conventionnés): part of the contributions is covered, depending on the sector and the conditions of the agreement.

Where do doctors pay pension contributions?

To the CARMF, the doctors’ pension fund, for basic and supplementary pensions and, for doctors under the agreement, the additional old-age allowance (ASV).

Does the simulator suit locum doctors?

Yes, as long as the activity is carried out on a self-employed basis, under non-commercial profits.

Does practising through a company change contributions?

Yes. In a professional practice company, contributions depend on pay and, where applicable, on part of the dividends. This simulator does not cover that case. See director’s pay

Are the results reliable?

The simulator is kept up to date with French social security rules by Urssaf. It gives an estimate: Council fees, insurance contracts and your personal situation can change the final figures.
